         Head Advisors are responsible for chartering their club, securing an EIN for their club, and annually filing FORM 990.  Please call Janice Hanna at 330-533-5538 if you have any questions with these procedures.

Other forms for Volunteers:

Club financial summaries for 2012 are due January 31, 2013.  You must complete a yearly club summary even if your club has no money.

Dog Enrollment Information new in 2013

ALL youth enrolled in 4-H dog projects are required to sign this document, as well as a parent or guardian is required to sign the document. This document must be signed each year a youth enrolls in a 4-H dog project and is to be kept on file at the county Extension office.
